Dr. Thiago Bianco Leal
Founder and lead surgeon·Thiago Bianco Clinic (legal entity: Bellegarde & Bianco Medicina Capilar Ltda)
Brazil
Background
Dr. Thiago Bianco Leal is a general surgeon who, per an independent ISHRS bio, trained in Europe to learn FUE before it was practiced in Brazil and was among the first Brazilian surgeons to offer it 1. His clinic site echoes this, saying he studied FUE in Spain and introduced it in Brazil afterward 2. The clinic, legally Bellegarde & Bianco Medicina Capilar Ltda, is based in Alphaville, Barueri, in greater Sao Paulo, and states that Dr. Thiago personally performs every transplant himself 3. He describes the goal of surgery as: "We want to bring back your self-esteem, not turn you into someone else" 2.
Procedures and techniques
The clinic's core offering is FUE delivered through what it calls the Thiago Bianco Method: proprietary mapping software calculates how many follicles the donor area can supply before extraction, and Dr. Thiago designs the hairline himself during planning 4. It reverses the usual order, cutting recipient-area micro-incisions before follicles are extracted, to shorten their time outside the body 5. Its Ultradensity option reaches up to 90 to 100 grafts per cm2 depending on donor profile, which Dr. Thiago attributes to customizing his own blades and buying a hyperbaric chamber 2. The clinic also offers beard and mustache transplants for correcting bald patches or designing a facial hairline, no-shave eyebrow transplants using donor hair from the scalp or elsewhere, female pattern transplants, and non-surgical hair restoration run by its trichologists and nurses 3.
Booking and aftercare
The published process opens with an evaluation consultation for feasibility, followed by preoperative exams (blood count, coagulogram, serology, electrocardiogram with cardiologist clearance); the clinic's own trichologist can address hair conditions beforehand if needed 5. Surgery lasts 8 to 10 hours 5. Afterward, patients go to a recovery area for a light meal before release the same day, with instructions to shower without washing the head and sleep on the stomach 5. Patients return the next day for a first hair wash, and those with high-density grafts are sent to a hyperbaric chamber to speed recovery 5. Follow-up visits are scheduled at 3 months, 6 months, and 1 year 5.

OP (u/Altruistic_Lie_4295), age 28, had a 3,450-graft Ultradensity FUE procedure with Dr. Thiago Bianco Leal in 2025, paying $18K USD (later citing 110,000 BRL, which he says came to over $20k after a worse exchange rate). At his 14-day update he said he 'really like[d] the design of the hairline' and reported normal healing. By his 6-month update his tone had shifted sharply: he called himself 'Extremely unhappy with the density so far,' reported overharvesting with visible scalp on the right side, suspected the result 'looks like the work of his assistants,' and doubted further improvement was coming — though he still liked the hairline design.

FUE3300 had a hairline-lowering/forehead-reduction procedure (he states he had no actual hair loss) with Dr. Thiago Bianco Leal in Brazil, 3361 FUE grafts, paying about 10,000 USD after a roughly year-long waitlist, with surgery around September 2021. Across updates from 2 weeks out through the 3-year mark he consistently reported being happy, calling the result 'a work of art' and saying he'd 'always repeat' the surgery with Bianco, while noting some early shock loss that grew back and a very long clinic waitlist. In numerous later recommendation threads through 2025 he continued to endorse Bianco as among the best in Brazil or the world for high-density and temple work, consistently disclosing his own bias as a patient.

u/jiriwelschsburner's first transplant was done at Thiago Bianco's sister clinic by a different doctor and left them dissatisfied with density; they then scheduled a second, 2,000-graft procedure with Thiago Bianco personally, done in 2025 for about $11,000 (discounted due to the earlier surgery). At 9 days post-op they rated the experience '10/10... from beginning to end incredible' and said they were 'highly confident' in the result. By an 8-month update they still hoped for more density in the front and middle, but concluded Bianco is 'deff a top surgeon who knows what he is doing.'

OP (25M) had an Ultra-Density FUE procedure with Dr. Thiago Bianco Leal in the São Paulo area in 2025, receiving ~3,400 grafts (7,200 hairs) to lower a naturally high hairline 1.5cm and fill thin temples, paying ~$18,000 USD. Across four posts made within days of each other, all around 12 days post-op, he consistently said he was 'very happy with the result' and called Dr. Bianco and his team 'the definition of world-class,' citing a no-pressure consultation, a highly responsive WhatsApp-based team, and the surgeon personally designing his hairline and checking in on him after surgery. No complications or negative experiences were reported.

DannyB331 had a repair transplant with Thiago Bianco Leal on 2025-11-11 (2,800 grafts, ultra-density technique) to fix an 'awful' prior transplant done by an unnamed doctor in America, paying Bianco Leal's usual $18K+ rate. He said the hairline was drawn based on other work of Bianco Leal's he liked, and they reviewed the design together multiple times before surgery. He called the outcome 'perfect and soooo dense' and 'worth it.' A follow-up existed as of April 2026 but no further outcome details were given in the available text.

random8002 had FUE with Dr. Thiago Bianco Leal (3,160 grafts, 6,746 hairs per the clinic), paying $16,222.42 USD total; at 118 days (3.9 months) post-op they reported the result positively despite commenters calling the price high. They found the clinic 'hospitable, professional, detail oriented' and said they see 'nothing wrong with going to a reputable, world class surgeon, and paying for it,' though they wished more grafts had gone to the temple corners/widow's peak. In an earlier comment on another patient's post about the same clinic, they affirmed that account was accurate, having 'gone to the same clinic' themselves.

- surgery 2026
mallomar chose Dr. Thiago Bianco over Dr. Carlos Wesley (NYC) for temple/hairline FUE, quoted roughly $18,700-$19,550 for about 3000 grafts with a non-refundable $850-1000 deposit; surgery took place in early February 2026. One week post-op, they reported no head shave, with grafts placed mainly in the temples, some corners, crown-midline density, and a minimal hairline, and said a fuller update would come around the four-month mark. In an earlier comment on another patient's 6-month Bianco update, mallomar noted Bianco changed his post-surgery medication plan, skipping tadalafil because of their minoxidil use and low-normal blood pressure. No overall satisfaction verdict has been given in any of these reports.

At 5 months post-op he said, 'If you saw me right now you’d be hard pressed to believe I had a transplant.' By 6 months he called himself 'beyond grateful and thrilled with my result' and 'super happy with my result and very grateful to Dr. Bianco and his entire team, they’re world-class.' At his 9-month update he shared photos after his first real haircut, noting he could finally style over areas that once showed thinning, with no complications reported at any point.

u/fasfkre had Dr. Thiago Bianco Leal perform a 3,940-graft procedure in 2025 (673 single/1,922 double/1,339 triple hair grafts, density 70 front/40 temples/40 crown). Before going ahead, they wrote 'I was very disappointed with Thiago Bianco communication.' By day 1 post-op they had not given a result verdict, instead asking the community how the diffuse thinning coverage behind the hairline and toward the crown looked, since no prior patient diaries had shown this surgeon doing that kind of work.

u/ScottyTooHotty98 had a hair transplant with Dr. Thiago Bianco Leal on 2026-01-29, receiving a total of 2901 grafts to correct congenital triangular alopecia in the middle of his hairline; no technique or price was stated. He posted photos on Day 1, before his first post-op wash, and said he'd keep posting updates over the following months, but gave no explicit verdict on the surgeon or result, and a commenter's request for an update in June went unanswered in the thread.

- 3,500 grafts
u/TreadTheEarth posted in June 2024 while deciding between two surgeons for a hairline-with-temple-points design: Dr. Pekiner (~2,500 grafts, ~3 euro/graft, ~$8k) versus Dr. Bianco Leal (~3,500 grafts, flat quoted fee of ~$18,000 USD). They leaned toward Leal's flat-fee structure, reasoning that opting for his higher-density option would look good without adding cost, but flagged the scarcity of reviews on Leal as a downside in weighing the decision. As of this post nothing had been booked and no surgery had taken place with either surgeon.

jose-baldo has not had surgery with Thiago Bianco Leal, but weighed in on a thread comparing surgeons for natural hairline and temple-point work. They said Thiago 'is also very good with temple points, but hes super aggressive,' adding he 'only really knows how to make a super low Zalyn Mayik type hairline' and is a fit only 'if thats your thing.' They also noted he 'can also graft at very high density,' though said they were 'not sure if its worth it.'
